Inner South East Post Acute Care
The Inner South East Post Acute Care (ISEPAC) program aims to support patient's recuperation after a hospital episode, via the provision of individually tailored packages of community based care services.
PAC arranges support services in a timely, responsive manner and respects cultural values.
The program promotes health and independence whilst providing a vital link to ongoing community supports.
Where another program was providing supports prior to the hospital episode it is expected that these services will be resumed on the person's discharge to home and PAC can supplement that service.

Description of PAC support services include:
The ISEPAC program, as part of the Inner South Community Health Service, provides services to all public hospital patients residing in the municipalities of City of Port Phillip, Stonnington, Glen Eira and Bayside.
ISEPAC coordinators are based in the Alfred Hospital, Caulfield Hospital and Head Office.
How to refer to Post Acute Care?
Contact the ISEPAC Office in relation to what hospital the patient is being discharge from. All hospitals other that The Alfred and Caulfield Hospitals contact Head Office.
