The HARP Disease Management Unit is the ambulatory care wing of the Medical Unit (GMU) and provides comprehensive care and proactive follow up to patients with chronic medical conditions who are at risk of hospitalisation.
This is done through specialised clinics, care coordination and patient education.
Enrolled patients are reviewed regularly by a Consultant or Registrar in a DMU clinic.
Each appointment is allocated 30 minutes to enable a comprehensive medical review and development of a care plan.
The GP is sent the consultation review on the same day.
The clinics are held in community and hospital locations. These include:
A pharmacist can review patients in The Alfred, Caulfield and Sandringham clinics or undertake home medication reviews.
Patients and their carers also have their care coordinated by the DMU Care Co-ordinators. A Care Coordinator will identify and coordinate the needs of the patient.
Other services include quarterly patient newsletters and group patient education sessions.
These sessions are provided by psychologists, OT's, dieticians and are designed to help patients manage their health.
They cover a range of topics such as anxiety and depression, energy conservation and nutrition.
Referrals can be made by faxing a patient summary to 9076 3812 or calling one of the care co-ordinators on 9076 2375.
