The GP Liaison team at Alfred Health aims to improve communication between General Practitioners and The Alfred to enhance patient care.
This includes:
The development and implementation of "Referral and Management Guidelines for General Practitioners" to assist in Outpatient demand management and facilitate patient care.
Facilitating communication between the hospital and GPs in the community, including HMO education and the development of electronic discharge summaries.
Website redevelopment and maintenance.
Assist GPs in accessing hospital services.
Liaising with GPs through General Practice Divisions on a number of projects, including GP and practice nurse education programs.
