Community Aged Persons' Mental Health Service

Description of Service

The Community Aged Persons' Mental Health Service forms part of the Aged Psychiatry Services at Caulfield Hospital.

The team is available to see people in their own homes; which may include a hostel, nursing home, and/or supportive residential service.

The team of Psychiatrists, Psychiatric Registrars, Psychiatric Nurses, Social Workers, Occupational Therapists and Psychologists works closely with the client's General Practitioner and family / carer.

Trained and approved interpreters will be used when assessing and treating non-English speaking clients.

The team will visit people who live in

  • Port Phillip
  • Stonnington
  • Caulfield section of Glen Eira

Service Provision Criteria
Persons 65 years or over who suffer from or experience a mental illness that requires specialist psychiatric assessment/management, such as:

  • Psychiatric Assessment: The team will conduct assessments in the community and offer advice, management, support, treatment and follow-up
  • Case Management
  • Carer and family support
  • Education to clients, carers and health workers who care for older people with mental health problems.
